LUMEDX USA is pleased to offer a comprehensive benefits program to its valued
employees.
Health and Wellness
LUMEDX offers a variety of important benefit programs to protect the wellbeing of you and your eligible family members, including domestic partners. We offer comprehensive medical, dental, vision, disability and group life insurance coverage for you, as well as voluntary coverage options for your family members.
Medical Insurance
LUMEDX provides two comprehensive PPO medical plans through Premera Blue Cross with low deductibles, co-pays, and prescription drug cost. These two plans are designed to offer our employees a choice in their medical plans to satisfy different needs.
Vision Insurance
Provided through Vision Service Plan (VSP), includes allowances for eye exams, hardware and contacts.
Dental Insurance
LUMEDX provides a dental plan through Washington Dental Services/Delta Dental which offers preventative, basic, and major services.
Personal Time Off (PTO)
This benefit combines traditional vacation and sick leave in one time off policy. The amount of PTO employees receive each year increases with the length of employment. Upon hire, PTO hours are accrued bi-weekly at 4.62 hours. The hours increase after 2 years, 10 years, and after 15 years.
Paid Holidays
LUMEDX provides 11 paid holidays per calendar year.
Long Term Disability
The Long Term Disability Insurance provided to LUMEDX employees allows them to receive 60% of their salary after 90 days of continuous disability. LUMEDX pays 100% of the insurance premium for this benefit.
Voluntary Short Term Disability
Provided by Colonial Supplemental Insurance, employee can elect short term disability benefits through payroll deduction. This benefit has a guaranteed issue, a monthly benefit up to $3,000, and is portable.
Life Insurance/AD&D
Provided by Sun Life, we offer $50,000 Basic Life policy, 100% paid by LUMEDX. Supplemental Voluntary Life is available at additional premium through payroll deduction.
Employee Assistance Program
LUMEDX provides a free and confidential service that connects employees and members of their household with experienced counseling professionals for short term help with personal problems, including stress management, legal or financial consultations. The Employee Assistance Program is available anytime, not just in an emergency, for a wide range of concerns including the problems of coping with daily life.
401(k) Retirement Plan
We contract with Kibble and Prentice to provide free financial planning to our employees and manage our 401(k) program.
Flexible Spending Account
This benefit allows you to lower your taxable income by reimbursing eligible out-of-pocket medical, dental, and vision expenses, including some over the counter medications, from payroll deductions on a pretax basis up to $3,000 per year. A Day Care plan is also offered for up to $5,000 per year.
Wellness Benefit
LUMEDX provides a discounted membership to 24 Hour Fitness. We also offer reimbursement to help offset monthly membership fees if the employee joins a fitness institution other than 24 Hour Fitness.
Public Transportation or Parking
LUMEDX encourages use of public transportation or other commuter transit options such as Vanpool, and sponsors the Commuter Bonus Transportation Benefit Program. The Commuter Bonus Program provides a non-taxable transportation subsidy (up to $50 per month) for employees to commute by bus, vanpool or ferry at least 51% of all workdays. Optionally, LUMEDX co-shares onsite parking costs at the rate of $25/month for those not participating in the public transportation reimbursement benefit. (Ask for additional details for employees based at our Oakland location.)
Jury Duty and Military Leave
LUMEDX allows up to 5 paid days for jury duty or military leave per calendar year.
Anniversary gift
LUMEDX recognizes service achievement milestones to employees reaching their 5, 10, 15, 20 and 25 year anniversaries with the company.
