Cardiac Transplant Module: Automate Donor / Recipient Record Keeping for Heart Transplant Centers
"LUMEDX's Cardiac Transplant Module enhances workflow, promotes data quality, and simplifies processing of UNOS and CTRD Registry information. Data is entered once in the system and integrates with other data found across the CV service line, helping to eliminate redundant data entry and manual errors, while supporting clinical research and patient care. It also produces downloadable forms for UNOS and CTRD Registry. LUMEDX carefully listened to our needs and performed as a true partner in helping deliver a Cardiac Transplant solution robust enough to meet our demands for today and for the future."

The Cardiac Transplant Module enables heart transplant centers to easily track adult cardiac transplant patient data from first consultation through evaluation, activation, transplant and post-transplant follow-up visits. The Cardiac Transplant Module also allows the heart transplant center to track all donation offers and actual donor information for each patient, enabling the center to meet government record-keeping requirements.
By automating, structuring and streamlining the process to collect information and record results, heart transplant centers can track, understand, and manage outcomes, as well as document quality of performance to patients, physicians, hospital administration, insurance carriers, and government.
The Cardiac Transplant Module is an integrated part of the Apollo Advance Cardiovascular Data Repository, CardioManager and other LUMEDX products. This robust integration produces a comprehensive patient record as heart center staff can seamlessly access, view, and manage all events that have occurred across the cardiovascular service line.
Benefits:
Helps automate and streamline workflow to track both recipients and donor information - fully integrated solution decreases data entry and manual search efforts
Improves patient safety and decision-making - all recipient information is available in one central location for easy access by physicians and staff
Helps satisfy government regulations for tracking and documenting all donation offers
Promotes more efficient and accurate UNOS data collection by pulling all required data from just one source - eliminates hunting through patient charts and performing added manual data entry
Ensures robust data collection by tracking follow-up information on all cardiac events, biopsies, infections and rejections
Promotes quality of data - importing data automatically from prior visits and using a standard data collection template help eliminate gaps, errors and efficiencies associated with hand-written notes, paper-based charts, and redundant manual data entry
Facilitates communications with referring physicians by providing an easy-to-use Referring Physician Lookup feature that tracks the contact information of thousands of referring physicians
Monitors ventricular assist device implantations throughout continuum of care
Provides transplant centers with the power of real-time information to manage their statistics and outcomes, and the insight to improve performance in critical areas, including:
